Process of pregnancy care
Antenatal care is a complex health service based on the cooperation of the health visitor, the general practitioner, the obstetrician-gynaecologist, the midwife of the expectant mother’s choice and the mother to be. It is the process of care from the moment the pregnancy is confirmed until the beginning of labour, or the pregnancy is terminated. Antenatal care begins when the obstetrician-gynaecologist confirms a live intrauterine pregnancy, carries out a risk assessment and issues a certificate to the expectant mother.
Confirmation of pregnancy
To confirm pregnancy, a visit to an obstetrician-gynaecologist is required after the first signs of pregnancy (no menstruation, breast tenderness, morning sickness, nausea, sensitivity to smells, tiredness) and if the result of a home pregnancy test is positive. During the obstetrician-gynaecologist consultation, after a thorough questioning, the specialist will confirm the pregnancy with vaginal ultrasound scan and will determine the age of the foetus and the expected date of delivery based on the embryo’s size and the date of the last menstrual period.
Stages of pregnancy
The 40 weeks of pregnancy are divided into 3 trimesters. Each trimester has its own physiological and psychological changes that prepare the female body for motherhood. To monitor these changes and the development of the foetus, antenatal care involves specialist consultations, laboratory tests and pregnancy ultrasound examinations at specified intervals. At weeks 1 to 12, it is recommended to have a genetic ultrasound scan in the first trimester to screen for Down’s syndrome. At 13-27 weeks, the second trimester genetic ultrasound scan is performed, where the foetus’ body contours, facial and cranial scans and limbs can be seen. Foetal ultrasound is then recommended at 30-32 weeks.

The first obstetrician-gynaecologist examination
If possible, it is recommended to have the first OBGYN examination performed by the 12th week of pregnancy. This examination is the basis for admission to antenatal care and is the primary time for determining the expected delivery date, including confirmation of intrauterine pregnancy by bimanual examination, vaginal ultrasound, possibly laboratory pregnancy test, colposcopy (if a cytology sample is due), breast examination, weight and height measurement, and counselling.
1st trimester laboratory test items
Blood count (haemoglobin, haematocrit, erythrocyte indices, white blood cell count, platelet count), renal function (Se creatinine, urea, eGFR), liver function (GOT, GPT, Se bilirubin), blood sugar, thyroid hormone function (TSH), blood typing and red blood cell antibody screening, Treponema pallidum screening (TPHA+RPR), HbsAg screening, complete urine and sediment (protein, pus, glucose, acetone, UBG, sediment).
Resting ECG
The resting ECG test does not require any preparation. During the examination, the patient lies in a supine position and electrodes are placed on the torso and limbs. The test provides a picture of the electrical activity accompanying the heartbeat and its changes. The ECG curve is displayed on a monitor, which is also printed out as curves drawn on a paper strip. You cannot move or speak during the test and must remain still throughout.
Internal medicine specialty consultation (the examination can be carried out with laboratory test and resting ECG results):
General internal organ assessment: family and individual medical history, predisposing and risk factors, possible complaints. General physical examination, including assessment of the condition of the major organ systems (skin, mucous membranes, lymph nodes, respiratory system, cardiovascular system, abdominal organs, musculoskeletal system, and nervous system), blood pressure, BMI measurement, lifestyle advice. A detailed evaluation of the previously performed resting ECG is given.
2 obstetrician-gynaecologist examinations per trimester
During the examinations, body weight, pulse, and blood pressure are measured, the position of the foetus, foetal movement, foetal heart function, uterine fundus, uterine contractions are determined, and, if necessary, vaginal exploration, a vaginal examination, a cervix score if needed, the week of pregnancy is determined, and a urine test strip (protein, glucose-acetone, pus) is examined if necessary. Checking foetal heart function from week 28, teaching pregnant woman how to count foetal movement from week 34. Counselling, discussion of complaints, treatment if necessary, setting the date for the next check-up.

Combined test (free beta-HCG, PAPP-A)
The combined test is used for biochemical screening of foetal abnormalities and to assess foetal risk, which can more accurately determine the likelihood of certain foetal abnormalities, including trisomy 21, Down’s disease (85-90%), Edwards disease (trisomy 18) and Patau’s syndrome (trisomy 13).

2nd trimester laboratory test items
Blood count (haemoglobin, haematocrit, erythrocyte indices, white blood cell count, platelet count), complete urine (protein, pus, glucose, acetone, UBG, sediment), stress blood glucose test (fasting and 120 minutes after 75 g glucose consumption).
3rd trimester ultrasound examination elements

3rd trimester laboratory test items
Blood count (haemoglobin, haematocrit, erythrocyte indices, white blood cell count, platelet count), renal function (creatinine, urea, eGFR), liver function (GOT, GPT, Se bilirubin), coagulation parameters (INR and fibrinogen), complete urine and sediment (protein, pus, glucose, acetone, UBG, sediment), blood type and antibody screening, vaginal secretion culture (group B Streptococcus screening).
